This is a program that includes 100% of your preventative maintenance care AND discounts on other dental services in office! This is designed for those who DO NOT have any dental insurance.

New Patient Adult Plan

For 13 years and older — $449.00 per year

• 2 Doctor Exams (comp/periodic)
• 2 Dental Maintenance Cleanings (excluding SRP)
• 2 Fluoride Treatments
• All necessary x-rays (excluding CT scans)
• $250.00 Discount on SureSmile Ortho treatment & Sleep guard treatments
• $75.00 Discount off Sleep Laser
• 15% Discount on additional dental restorative and perio services

Adult Renewal Plan

For 13 years and older — $399.00 per year

• 2 Doctor Exams (comp/periodic)
• 2 Dental Maintenance Cleanings
• 2 Fluoride Treatments
• All necessary x-rays (excluding CT scans)
• $250.00 Discount on SureSmile Ortho & Sleep guard treatments
• $75.00 Discount off Sleep Laser
• 15% Discount on additional dental restorative and perio services

Child Plan

For 13 years and under — $299.00 per year

• 2 Doctor Exams (comp/periodic)
• 2 Dental Maintenance Cleanings
• 2 Fluoride Treatments
• All necessary x-rays (excluding CT scans)
• $250.00 Discount on SureSmile Ortho & Sleep guard treatments
• 15% Discount on additional dental restorative and perio services

Terms and Limitations

Rates are subject to change annually. This is an annual plan. Plan begins the date of the plan fee is paid and ends one year later.  It does not renew automatically. Program fee is due in full at registration and is non-refundable. Plan is non-transferable. Services cannot be billed to ANY insurance. Care Credit cannot be used to purchase plan. Care Credit can be used to pay for restorative work but will include a 10% discount, not the 15% discount. Use it or lose it, services do not accumulate. This plan is only good at Ledgeview Dental only; therefore, if you are referred to a specialty office, LLP discounts will not apply. It is the patient’s responsibility to schedule and keep appointments in order to stay within the plan year.
