The Ledgeview Loyalty Plan includes 100% of your preventive maintenance care along with discounts on additional in-office dental services. It is designed for patients who do not have dental insurance.

Annual Membership

New Patient Adult Plan

For ages 13 and older

$449 per year

Two comprehensive or periodic doctor exams
Two dental maintenance cleanings, excluding scaling and root planing
Two fluoride treatments
All necessary X-rays, excluding CT scans
$250 discount on SureSmile orthodontic treatment and sleep guard treatment
$75 discount on Sleep Laser treatment
15% discount on additional restorative and periodontal services

Annual Membership

Adult Renewal Plan

For ages 13 and older

$399 per year

Two comprehensive or periodic doctor exams
Two dental maintenance cleanings
Two fluoride treatments
All necessary X-rays, excluding CT scans
$250 discount on SureSmile orthodontic treatment and sleep guard treatment
$75 discount on Sleep Laser treatment
15% discount on additional restorative and periodontal services

Child Plan

For ages 13 and under

$299 per year

Terms and Limitations

Rates are subject to change annually. This is an annual plan beginning on the date the plan fee is paid and ending one year later. The plan does not renew automatically.

The program fee is due in full at registration and is non-refundable. Membership is non-transferable, and services received through the plan cannot be billed to dental insurance.

CareCredit cannot be used to purchase the plan. CareCredit may be used for restorative treatment, but treatment paid through CareCredit will receive a 10% discount rather than the standard 15% plan discount.

Unused services do not accumulate or carry over into another plan year. The plan is valid only at Ledgeview Dental Care. Discounts do not apply to services provided by an outside specialty office.

It is the patient’s responsibility to schedule and keep appointments within the active plan year.
